First Appointment Information

In preparation for your first visit to our clinic, we want to make sure you are ready and know what to expect.
 
Arrival:
Please arrive 10 -15 minutes before your scheduled initial evaluation appointment.
 
What To Bring:

  • Your insurance card, medication list, and photo ID. We will make a copy of these documents for your chart.
  • Your prescription for physical therapy from your physician if it is required by your insurance company.
  • We will send you specific forms to fill out ahead of time in order to better coordinate your care. Please bring these forms with you.
    • These forms include:
      • medical history form
      • personal information form
      • policy regarding cancellations and no-shows
  • Any applicable co-pay or estimated coinsurance

If you speak a language other than English, please bring a translator with you. Many of our clinics have team members who speak additional languages who can make your visit smoother. Please inform our clinic prior to arrival if you require language assistance.

What to Wear:
During your visit with the therapist, we ask that you to dress in comfortable, loose clothing, ideally shorts/pants (non-jeans) and a t-shirt.  Clothing that does not restrict movement and that allows visualization of the problem area is recommended. Sturdy shoes are also recommended- flip flops, slippers or heels should be avoided.

What Happens When I see the Physical Therapist?
Your therapist will work closely with you to improve your ability to move, improve your ability to communicate, reduce or manage pain, restore function, improve performance and/or prevent disability. During this initial evaluation, the therapist will examine you, discuss findings in detail and provide a mutually agreed upon treatment plan to achieve your goals. The treatment plan includes the anticipated length of treatment and recommended interventions that will be most beneficial- these may include a variety of hands-on techniques, education, modalities, and activity-based interventions.

New Patient Forms: 

Once you are scheduled for your initial evaluation, you will have new patient paperwork that will need to be filled out prior to your first visit.

You can access your patient paperwork through the portal. Prior to your first visit, you will receive a temporary password to access your patient portal. If you have forgotten or lost your patient portal password, you can request a new one by selecting the Patient Portal link in the upper right corner.
